Comparative study of histopathological diagnostic criteria in cutaneous lesions of lichen planus and discoid lupus erythematosus

Background and aims: Lichen planus (LP) discoid lupus erythematosus (DLE) are two relatively common mucocutaneous lesions whose clinical histopathological features overlap in some cases. The present study aimed to distinguish between these histopathologically order treat them more accurately. Methods: In a cross-sectional descriptive-analytical study, 29 48 microscopic slides of skin samples DLE LP, respectively, were examined the pathology archive Al-Zahra hospital Isfahan from 2008 2018. prepared by hematoxylin-eosin staining simultaneously blindly three pathologists with light microscope compared according certain criteria. Then obtained data analyzed SPSS version 24 using chi-square, Fisher’s exact, Mann-Whitney, t tests (P<0.05). Results: Based on findings, presence hyperparakeratosis superficial hyperorthokeratosis, epithelial atrophy, deep perivascular infiltration, edema papillary dermis, plasma cells lymphohistiocytes inflammatory mucin dermis significantly higher than LP On other hand, intensity lichenoid saw tooth hyperplasia rete ridges, cleft epithelium connective tissue, spongiosis, hyperorthokeratosis alone, wedge-shaped hypergranulosis Conclusion: Perieccrine perifollicular inflammation, Civatte bodies (CBs), abundance fibrosis, pale keratinocytes, pseudoepitheliomatous not criteria for differential diagnosis DLE.
